The Ministry of Revenue & Duties explains
The Holosiiv District National Tax Inspectorate at the Directorate General of the MRDU in Kyiv city explains that to ensure the rights of individuals, who dwell (registered) in the Autonomous Republic of Crimea and Sevastopol city, to administrative services as well as the rights as the citizens of Ukraine to get a tax number assigned or be registered as a taxpayer under the passport series and number, the Ministry of Revenue and Duties of Ukraine, in its letter of 24.03.2014 under No 6681/799-99-18-02-01-17, it has placed the regional authorities of the MRDU under obligation to receipt the above said citizens at any MRDU’s regional outlet.
Thus, individuals, who reside (are registered) in the Autonomous Republic of Crimea and Sevastopol city, can appeal to any regional office of the MRDU and file registration application form №1ДР, Notification № 1П (for natural persons who, due to their religious convictions, refuse to be assigned a tax identification number), application form №5ДР/5ДРП, as well as get a document that proves registration, whereof a record is to be entered in the national registry, can apply to any regional unit of the MRDU.
A form to register a child under age of 16 or minor who has not got a passport is to be filed by one of the parents (adopter, guardian, custodian) having the birth certificate and documents certifying identity of one of the parents (adopter, guardian, custodian). If the language of the birth certificate issued is other than Ukrainian, it is required to attach the Ukrainian translation of the birth certificate that is certified (original document will be returned after it is shown) and its copy (par. 5 part VII of the Regulations on Registration of Individuals in the National Register for Individual Taxpayers, which were adopted by the Ministry of Revenue and Duties of Ukraine on 10.12.2013, Resolution No 779).
